Risk Assessment & Management

The WHO conceptual framework for the international classification for patient safety offers a definition of Safety & Risk Management (SRM) as follows: “Activities or measures taken by an individual or a healthcare organization to prevent, remedy, or mitigate the occurrence or reoccurrence of a real or potential safety event.” Institute of Medicine (IOM) describes 'Patient Safety' as the freedom from accidental injury due to medical care or from medical error. An error is considered a failure to carry out a planned action as intended or the application of an incorrect plan. At either the planning or execution phase, errors may manifest by doing the wrong thing (commission) or by failing to do the right thing (omission).

While the hospital setting is one of healing and comfort, it also includes certain dangers. Hospitals contain hazardous chemicals, drugs, radioactive material, and infectious matter. In addition, there are dangers from fire and smoke.

Hospitals should ask the following questions when we talk about risk management and assessment:

  • Has my organization done the Risk Assessment?
  • Is my organization prepared to respond to these events?
  • Does my organization need to be prepared anyway?
  • What are our priorities?

Risk is mainly categorized into these categories:

  • Naturally occurring events: earthquakes, floods, epidemics and pandemics.
  • Technological events: electrical failure, transportation emergency, water emergency, HVAC failure, structural damage, and fire.
  • Human related events: mass casualty incidents (trauma), Mass casualty incident (infectious), terrorism - biological, hostage situation, bomb threat, civil disturbance.
  • Events involving hazardous materials: Terrorism - chemical, chemical exposure radiological exposure

Resource Planning: Manpower / Inventory / Equipment Etc

The healthcare industry needs to focus on improving the quality of healthcare and operational efficiency while reducing costs and optimizing back-end operations. To maximize profits, reduce costs and achieve a seamless healthcare continuum, it is imperative to improve the efficiency of back-end business functions across supply chain management, inventory management, patient relationship management, human resources, finance and billing. It can be achieved by business process optimization and technology enablement.
